What is the Mode of Action of Oflosyn?

Oflosyn is Ofloxacin, a synthetic antibiotic belonging to a group called fluoroquinolones. Oflosyn is bactericidal. Oflosyn kills bacteria by inhibiting the enzyme responsible for DNA replication and transcription.

Oflosyn can enter cells easily and is usually used to treat intracellular infections of the lungs (pneumonia) and genito-urinary tract.

FORMULATIONS:
Each Tablet contains:
Ofloxacin ..................... 400 mg

PHARMACOKINETICS:
Ofloxacin is readily absorbed from the gastro-intestinal tract. Oral bioavailability of almost 100% and a peak plasma concentration of about 3 to 4 ug/ml is achieved after a 400-mg oral dose. Presence of food may delay the absorption. Reported plasma half-life is about 5 to 8 hours; half-life may be prolonged in severe renal failure. The drug is about 25% plasma protein-bound. Ofloxacin is widely distributed in the body. It is found in the CSF, it crosses the placenta and is distributed in breast-milk. High-concentration are achieved in the bile. Principal means of excretion is through the urine (about 75 to 80% of which as unchanged drug; less than 5% excreted as metabolites) and about 4-8% are eliminated in the feces.

INDICATION:
For the treatment of infection including anthrax, biliary-tract infections, infected bites and stings, bone and joint infections, brucellosis, cat scratch disease, chancroid, exacerbations of cystic fibrosis, gastro-enteritis, gonorrhea, legionnaires’ disease, otitis externa, otitis media, peritonitis, Q fever, lower respiratory tract infections, septicaemia, skin infections, spotted fever, typhoid and paratyphoid fever, typhus, urinary tract infections, meningococcal meningitis prophylaxis, surgical infection prophylaxis, chlamydial infections and in the treatment of mycobacterial infections such as leprosy. 

PRECAUTION:
Ofloxacin should be given with caution in patients with epilepsy or a history of CNS disorders. Treatment should be withdrawn if tendon pain, inflammation, or ruptures occur. Care is necessary if the drug is to be given to patients with impaired hepatic or renal function, glucose-6-phosphate, dehydrogenase deficiency, or myasthenia gravis. Adequate fluid intake should be maintained during treatment; excessive alkalinity of the urine avoided because of the risk of crystalluria. Exposure to strong sunlight and sunlamps should also be avoided. Patients are not advised to drive a vehicle or to operate machinery when alcohol is also taken.

CONTRAINDICATION:
Ofloxacin should not be taken to children, adolescent, pregnant and lactating women because the drug has been shown to cause degenerative changes in weight-bearing joints, and tendon damage may occur. The drug should be avoided in meticillin-resistant Staphylococcus aureus infections because of the high level of resistance.

DOSAGE:
All doses to be taken 1 hour before or 2 hours after meals.
Urinary Tract Infections: Adult – 200 to 400 mg (1/2 to 1 tablet) daily preferably in the morning increased if necessary in upper urinary tract infections to 400 mg (1 tablet) twice daily.
Chronic Prostatitis: Adult – 200 mg (1/2 tablet) twice daily for 28 days.
Lower Respiratory Tract Infections: Adult – 400 mg (1 tablet) preperably in the morning, increased if necessary to 400 mg (1 tablet) twice daily.
Skin and Soft Tissue Infections: Adult – 400 mg (1 tablet) twice daily.
Uncomplicated Gonorrhea: Adult – 400 mg (1 tablet) as a single dose.
Uncomplicated Genital Chamydial Infections, NonGonococcal
Urethristis: Adult – 400 mg (1 tablet) in single or divided doses for 7 days.
Pelvic Inflammatory Disease: Adult – 400 mg (1 tablet) twice daily for 14 days.
Or as prescribed by a physician.

ADVERSE EFFECTS:
Gastro-intestinal effects: nausea, vomiting, diarrhea, abdominal pain, dyspepsia, rarely Pseudomembranous colitis,
CNS- related effects: headache, dizziness, restlessness, tremor, drowsiness, insomnia, nightmares , visual and other sensory disturbances and more rarely hallucinations, psychotic reactions, depression, convulsions, occationally paraesthesia and peripheral neuropathy.
Dermatologic effects: rash, pruritus, rarely vaculitis, erythema multiforme, Stevens-Johnson syndrome, toxic epidermal necrolysis, photosensitivity
Hepatic effects: elevated liver enzyme values, jaundice, hepatitis
Hermatological effects: eosinophilia, leucopenia, thrombocytopenia, hemolytic anemia or agranulocytosis
Others: transient increase in serum creatinine or blood urea nitrogen, occasionally acute renal failure, crystalluria, tachycardia.

DRUG INTERACTIONS:
Theophylline: Concurrent administration of Ofloxacin with Theophylline may lead to elevated serum concentration and prolonged elimination half-life of Theophylline which may result to Theophylline-related adverse reactions.
Caffeine: Caffeine elimination half-life has been reported to be increased and clearance decreased by the concomitant administration of ofloxacin.
Antacids containing Mg, Al or Ca: Concurrent administration of these antacids may substantially interfere with the absorption of ofloxacin.
Phenytoin: Altered serum levels of phenytoin have been reported in patients concomitantly receiving Ofloxacin.
Warfarin: Quinolones have been reported to enhance the effects of warfarin and its derivatives.
Probenecid: Probenecid interferes with renal tubular secretion of Ofloxacin.
